Essential Records for Your Consultation
Having your medical papers in chronological order allows Dr. Nisshii Bhatt to quickly evaluate the complete trajectory of development, past interventions, and current neurological status.
1. Diagnostic Imaging & Neuro Tests
- • Brain & Spine MRI reports along with physical films or digital CD/drive links
- • EEG / Video-EEG tracings & neurologist evaluation notes
- • BERA, hearing screening, and ophthalmology / vision reports
2. Birth & Clinical History
- • NICU / Neonatal discharge summary (especially for preterm, jaundice, or HIE cases)
- • Paediatric milestone charts, growth records, and immunization cards
- • Genetic screening, karyotyping, or metabolic testing reports (if done)
3. Therapy Records & Home Notes
- • Past and current Physiotherapy, Occupational Therapy & Speech notes
- • Current prescription list with exact drug names and dosages
- • A written list of specific questions or new observations noted by parents
